2) You need singpass to logon
Owner’s NRIC (front and back) 27.4kb OR 284 PIXELS X 371 PIXELS Sterilisation certificate if your dog is sterilized (????) Microchip barcode if your dog is microchipped (????)
3) Submit the application and take note of the application number eg. A8200700042104
4) Wait couple of days and check application status
KNN !! I need to pay $25 for Dr.Phua to certify that the dog is sterilized and another $18 for Microchip Barcode. Appointment fixed on Monday 20 August 2007 @ 9 am . Serra attended to my query and she say maybe have to cut up the dog to reconfirm if dog is indeed sterilized. Since day one Lucky has not been menstruating .isn't that enuff ?? WTF
I have to set aside $150 for complete thhis Government requirement !
